As part of its World Health Day 2010 celebration, New Heights Community Health Centres would like to acknowledge the great work done by various community members to improve health in our communities. We are happy to issue a Call for Nominations for the Health Champions in our Community Award.

 All nominees will be honoured during the Health Champions in our Community ceremony at our World Health Day event on April 7, 2010, and featured on our web-site, in the event program and in the Health Champions in our Community video that will be screened at the awards ceremony. The grand-prize winner will be determined by a draw at the World Health Day event. 

 A Health Champion is someone who lives, works or goes to school in the New Heights CHCs catchment area (bounded by Eglinton Ave, Steeles Ave, Bathurst St. and Keele St.) and has made a significant contribution to protecting and promoting the health of community residents in line with the mission and values of New Heights Community Health Centres. Their contributions can include but are not limited to: advocating for new and/or better services in their community; raising  awareness about important health issues; promoting healthy lifestyles; creating community spaces and groups that help combat isolation and create a stronger community; initiating community projects that have a positive impact on the health of its residents;  providing support to marginalized and vulnerable populations (seniors, youth, disabled, newcomers, etc.); building community capacity through education and empowerment, etc.

 Health Champions can either nominate themselves or be nominated by other community members. To submit a nomination, please fill out the form and return it by mail, fax or in person to one of the addresses/fax numbers below:
