The Seniors Health Project - Aging at Home Program

What is the Seniors Health Project?

Seniors Health Project is a collaborative model of care which facilitates access to in-home health care and social services for vulnerable homebound seniors in the Englemount-Lawrence community.

  • Toronto Central Community Care Access Centre
  • Baycrest
  • Downsview Services to Seniors
  • St. Clair West Services for Seniors
  • Humber River Regional Hospital

What Services are Provided?

The Seniors Health Project facilitates access to in-home:

  • Multi-disciplinary primary care services (physician and nurse practitioner led)
  • Intensive case management
  • Toronto Central Community Care Access Centre services
  • Geriatric and psychogeriatric outreach/assessment teams
  • Specialized geriatric services
  • Range of community support services e.g. Meals on Wheels

 Eligibility Criteria:

Over the age of 65 years (or 55-64 if deemed marginalized)

  1. Difficulty accessing a family physician because of physical, cognitive, and/or psychiatric issues
  2. A valid OHIP card
  3. Live in the New Heights catchment area bound by: Steeles Ave., Eglinton Ave., Bathurst St and Keele St.
