Clients of the health centre see our staff by appointment. The length of your appointment will depend on your reason for visiting and so the receptionist will ask you a few questions when you call for an appointment. Each person must have a separate appointment. Talk to the receptionist and bring any information or records with you.
We believe that the best way to provide health care is to give appointments when they are needed. If you call, we will try to give you an appointment for later that day. If no appointments are available that day, you will be offered the next available appointment or the opportunity to speak to the triage nurse. Morning and evening appointments may be pre booked but because of demand, the wait to get a pre booked appointment may be longer. Monday’s are our busiest day and if you need a routine appointment we suggest calling on another day if you are able.
If you need to change or cancel your appointment, call us at (416) 787-1661. If you will be late, please call us. If you come late, you will have less time to spend with your nurse, doctor, dietitian, foot specialist, etc. If you arrive after the time that your appointment would be over, you will need to make a new appointment. Please arrive on time.
If you miss more than half of your appointment, you will be asked to rebook it and you will be marked as a “no show”. If you have 3 “no shows” you will be unable to receive clinical services until you have seen a case coordinator
One of our doctors is on-call for urgent medical problems when the health centre is closed. Call (416) 787-1661. You will get the answering service. Ask to speak to a doctor. The answering service will take your name and telephone number. Then the doctor will call you back. Please stay close to the telephone so the doctor can reach you. You must be a client of the health centre to use the on-call service.

If you have a serious emergency, like bad chest pains, heavy bleeding or sudden loss of control of your body, go directly to a hospital or call 911
All of our programmes and services are free. You do not need a health card to use them. If you do not have a health card, tell us and we may be able to help you get one. Do not allow anyone else to use your health card number. Also, do not use anyone else's health card number. Our clinical team members have to report any misuse of cards.
Home visits may be made to clients of the health centre who live in our catchment area and have health problems that prevent them from coming to the centre

Immigration physicals may be booked for existing clients of the health centre. Please use pre booked appointments.

Bring all your medications, vitamins and herbal remedies to your appointment.
Please call your pharmacy at least 48 hours before your prescription runs out to see if you need to have it refilled or ask your doctor, nurse or nurse practitioner during your appointment.

Do not call for test results. You will be called with any abnormal results or you can discuss your results with your provider at your next regular appointment.

Forms and correspondence must be completed/written during an appointment. They can no longer be dropped off for completion
